When to use this template

The psychiatric session note template is for clinicians documenting individual psychiatric or behavioral health sessions. The SOAP structure (Subjective, Objective, Assessment, Plan) is the standard progress-note format used by most mental health practices and EHRs. This template adds an AI Session Summary section at the top so a clinician can review the auto-generated narrative before signing off, and an Audit Trail section at the bottom so the practice can keep a defensible record of AI involvement in the chart.

Use it for individual psychiatry visits, behavioral health follow-ups, and any session where you need a structured progress note. Skip it for intake assessments (which need a longer biopsychosocial template) or for group therapy notes (which need a different structure).

How it differs from siblings

This is the only canonical Meetily template built for clinical workflows. Where the standard meeting template is generic, the psychiatric session template is specific to a regulated industry: it surfaces medications, diagnostic codes, safety assessment, and audit trail as first-class sections because all four are routinely required in chart audits. The AI Session Summary at the top is also specific - clinicians need a one-paragraph narrative they can quickly read and edit before signing, not a list of action items.

What the AI summary looks like

Meetily fills the AI Session Summary as one paragraph with a confidence score, then populates each SOAP section based on what the clinician and patient discussed. Medications and Diagnoses tables are conservative: they only capture entries that are explicitly mentioned in the session - the LLM does not infer medication changes from context. Safety & Risk Management is a paragraph that highlights any mention of suicidal ideation, homicidal ideation, or safety planning, flagged for clinician review. The Audit Trail is populated with the AI model version, timestamp, and a placeholder for the clinician to sign off with their name.
