When to use this template
The psychiatric session note template is for clinicians documenting individual psychiatric or behavioral health sessions. The SOAP structure (Subjective, Objective, Assessment, Plan) is the standard progress-note format used by most mental health practices and EHRs. How it differs from siblings

What the AI summary looks like
Meetily fills the AI Session Summary as one paragraph with a confidence score, then populates each SOAP section based on what the clinician and patient discussed. Medications and Diagnoses tables are conservative: they only capture entries that are explicitly mentioned in the session - the LLM does not infer medication changes from context. Safety & Risk Management is a paragraph that highlights any mention of suicidal ideation, homicidal ideation, or safety planning, flagged for clinician review. The Audit Trail is populated with the AI model version, timestamp, and a placeholder for the clinician to sign off with their name.
